We have a systematic process of collecting patient-specific information, assessing medication therapies to identify medication-related problems, developing a prioritized list of medication-related problems, and creating a plan to resolve them with the patient, caregiver and /or prescriber.

Customized interventions
Motivational interviewing: An interactive person-to-person consultation conducted between the patient and/or caregiver and the pharmacist and is designed to improve patients; knowledge of their prescription, over-the-counter (OTC) medications, herbal therapies and dietary supplements, identify and address problems or concerns that patients may have, and empower patients to self-manage their medications and their health condition(s). This method enhances our communication with patients and makes a difference in their health outcomes: Encourages patients to commit to taking medications according to their physicians' recommendations. We call backs to follow up with patients and confirm that they are taking the medications. Tracks all interventions, documenting prescribed treatments and patients' responses to motivational interviewing.
Desktop Comprehensive Medication Review (dCMR)
A CMR that is completed via the patient profile., We can either develop and implement a program for you.
Medication Action Plan (MAP)
This document is a care plan specific for our patient. It is patient centered and will help the patient focus on improving the self-management of their medication.
Personal Medication Record (PMR)
A comprehensive record of patient's medications including prescription, non-prescription medications, herbal products and other dietary supplements.
